Energy Healing Consent & Waiver
Description of Reiki: Reiki is a gentle, non-invasive energy healing technique that promotes relaxation, reduces stress, and supports the body's natural healing abilities. A Reiki practitioner channels universal life force energy through their hands to the recipient. Reiki is not a substitute for medical treatment or psychological counseling.
Client Acknowledgements and Agreement:
By signing below, I acknowledge and agree to the following:
Voluntary Participation: I am voluntarily participating in Reiki sessions provided by Embodied Resonance Healing Arts.
Not a Substitute for Medical Care: I understand that Reiki is not a substitute for medical diagnosis or treatment by a licensed physician or other qualified healthcare professional. I am advised to consult with my physician for any medical concerns I may have.
No Guarantees or Cures: I understand that there are no guarantees or cures, expressed or implied, regarding the outcome or effectiveness of Reiki treatments. Reiki can complement other healing arts but should not be relied upon as a primary treatment for any medical condition.
Information Provided: I have provided accurate and complete information about my health and well-being to the practitioner to the best of my knowledge. I understand that it is my responsibility to inform the practitioner of any changes in my health condition.
Physical Contact: I understand that the Reiki practitioner may place their hands gently on or slightly above my body during the session. I have the right to request that the practitioner not touch me at any time.
Emotional Release: I understand that Reiki may sometimes facilitate the release of emotions. I take responsibility for my emotional responses during and after the session.
Confidentiality: I understand that this is not a HIPAA-compliant facility. Any personal information I choose to share is voluntary and will be treated with respect and care. I understand that the practitioner will maintain the confidentiality of the information I share amongst the Embodied Resonance team, except as required by law.
Assumption of Risk: I understand that while Reiki is a gentle practice, there is a possibility of unexpected physical or emotional sensations. I voluntarily assume any and all risks associated with receiving Reiki treatments.
Consent & Session Approach: All healing modalities used—such as Reiki, sound, movement, or oracle cards—are guided by your consent and needs. Not every method will be used or expected in every session.
For Yoga Sessions: By participating in any yoga class, you acknowledge that you are doing so voluntarily and at your own risk. Please consult your healthcare provider before beginning any physical activity. You agree to listen to your body, practice safely, and inform the instructor of any injuries or conditions. The instructor and business are not responsible for any injury or harm that may occur during or after the class.
Release of Liability:
I, the undersigned client, hereby release, waive, discharge, and covenant not to sue Embodied Resonance Healing Arts, its owners, employees, practitioners (including but not limited to the named practitioner above), agents, and representatives (collectively referred to as "Released Parties") from any and all liability, claims, demands, actions, 1 and causes of action whatsoever arising out of or related to any loss, damage, or injury, including 2 but not limited to physical, emotional, or financial injury, that may be sustained by me while participating in or as a result of receiving Reiki treatments from the Released Parties. I understand the practitioner reserves the right to pause or end any session if they feel uncomfortable or if safety concerns arise, and may refuse service to anyone at their discretion.
This release shall be binding upon my heirs, executors, administrators, and assigns.
By checking the associated box during booking, I acknowledge that I have read and understand the Reiki & Energy Healing Waiver and Release of Liability, and I agree to proceed under these terms.
